I appreciate the report! This bug is just a graphical issue with no actual implication on progress or gameplay. It's caused by internet instability, so if you run into it again you'll know not to worry.

Sorry you ran into this issue! This looks like a general connection error making it appear as if your story progress was reset. Could you try fully closing out of the game and checking again? It should refresh everything, and your progress will appear as normal again.

Sorry you ran into this issue! Error code 8.UDE.12 means that the Relic you were attempting to open was invalid. What happened was that you actually encountered a visual bug in which a specific Relic duplicated itself, but since the duplicate relic doesn't actually exist, it couldn't be opened...
